What to include

- Specific actions or follow-up tasks for the incoming shift
What not to write
- Grouping all residents into one sentence โ each person's experience needs to be individually recorded
- Only documenting incidents โ 'settled night, no concerns' is also important to record
- Vague terms like 'fine' or 'okay' without any supporting detail

- Do I need to write a note if the whole night was uneventful?
- Yes. A record that states the night was settled and uneventful is valuable โ it shows the shift was covered, residents were checked, and nothing was missed. Gaps in the notes can raise questions during audits or incident reviews.
- What if a resident does not want their personal care noted?
- You can document support in general terms without unnecessary personal detail. For example, 'personal care support provided at 0300 hrs, resident resettled independently' is sufficient. Follow your organisation's privacy and dignity guidelines.
- Can I write one shift note covering both residents?
- Best practice is to document each resident separately, or to clearly label each section with the resident's name or identifier. Mixing residents in a single paragraph can create confusion if a note is ever reviewed in isolation.
